Southpointe Transitional
Living Services
The Mary Lee Foundation was innovative
in its approach to community-based programs for
people with disabilities. The Foundations development of
the Transitional Living Services program demonstrated a
successful alternative to institutionalization. The goal
of the program is to provide the supports necessary to assist a
person in maintaining their independence while always striving
to achieve total independence. The program offers room and
board, 24-hour supervision, supervised medication
administration, transportation and case management. The
program is located in a cluster of apartment complexes in Austin
Texas and serves a wide range of disabilities.

Intermediate
Care Facility for People with Mental Retardation or Related
Conditions (ICF-MR/RC)
The ICF-MR/RC
program is a Medicaid program that provides intense supervision
in a residential setting while providing highly structured
programs designed to increase a person's independence.
Each person's program is monitored by a Qualified Mental
Retardation Professional responsible for insuring all the needs
of the individual are met whether it be skills training,
habilitative services, medical services, or adjunctive
therapies.
Southpointe Home & Community-Based Services (HCS)
The Southpointe
Home & Community-Based Services program provides support to a
person in their home or in a supervised living situation.
Services provided to eligible persons include: foster
care, supervised living, day habilitation,
supported employment, speech, physical and occupational therapy,
psychological and social work services, nursing,
dental, adaptive aids and minor home
modifications.
