Rehabilitation details and requirements
We offer outpatient health care services that help you keep, get back, or improve skills and functioning for daily living that have been lost or impaired because of sickness, accident, or disability. Services include physical and occupational therapy and speech-language pathology. We work with your physician to create a rehabilitation plan just for you.
Start Therapy
To start therapy at the Mary Lee Foundation Rehab Center, call or email the admissions and billing coordinator at (512) 443-5777 . They will send you an intake form to fill out and will complete prior authorization with your insurance provider. Please provide a copy of the doctors referral or request a doctors referral to be sent to the Mary Lee Foundation Rehab Center at fax number (855) 978-1775. Once we receive these items a therapist will call you to schedule an initial evaluation.
