Variety of programs for people that need it the most

With flexible housing supports, people with disabilities can live just like any other member of the community. We operate three licensed Intermediate Care Facilities, several group homes and a long-term care housing program for adults with mental and/or physical disabilities there are many choices that allow your adult to live the independent life that they deserve. We can help you navigate this transition.

Daybreak

The Daybreak program is designed to provide supervision and activities for adults with disabilities. Our focus is on community integration, teaching independent living skills and job skills.

John Hartman Care Facilities (JHCF)

The John Hartman Care Facilities program provides a successful alternative to institutionalization for adults with disabilities.

Home & Community Based Services

The HCS program provides individualized services and supports to persons with intellectual disabilities who are living with their family, in their home or in other community settings, such as small group homes
